Usefulness of information from x-ray request forms to the radiologists at radiology department, Nnamdi Azikiwe University Teaching Hospital, Nnewi.

Abstract

This is retrospective analytical study done in the Radiology department of Nnamdi Azikiwe University Teaching Hospital (NAUTH), Nnewi on the usefulness of information from X-ray request forms to the radiologist. The study shows that little attention is paid to request forms. 200 Xray
forms were analyzed in a random sample. Some of the important information often neglected, according to the study include: Age-78 (39.0%) out of the 200 forms had ages in full; Address-65 (32.5%) had addresses in full but, information like surname (100.0%), other names (99.5%) and sex (98.0%) are almost always properly supplied. 0
