Tumeur du ligament large : à propos d’un cas

Abstract

L’objectif de cette étude était de décrire notre démarche diagnostique et thérapeutique devant une tumeur du ligament large de l’utérus chez une jeune patiente. Il s’agissait d’une patiente de 20 ans admise pour une masse abdomino-pelvienne évoluant depuis une césarienne pour dystocie mécanique. L’examen clinique et la tomodensitométrie objectivaient une masse molle, donnant une hernie ischiatique. L’exploration chirurgicale notait une masse bilobée du ligament large prenant l’utérus en sandwich. La section du pont unissant les deux lobes, ainsi qu’une installation en double équipe nous ont permis d’extraire la masse et conserver l’utérus. Les suites ont été simples. Avec un recul de 11 mois, la patiente se porte bien et est à sa 17ème semaine d’aménorrhée gravidique. Les tumeurs du ligament large sont rares et essentiellement bénignes. Le défi chez les jeunes femmes reste la préservation utéro-annexielle.

English Abstract

The objective of this study was to describe our diagnostic and therapeutic approach in front of broad ligament tumor in a young patient. There was a 20 year old patient admitted for abdominalpelvic mass evolving for a caesarean section for obstructed labor. The clinical examination and CT objectified a soft mass, giving a sciatic hernia. Surgical exploration noted a lobed mass of taking the uterus broad ligament sandwich. The section of the bridge linking the two lobes, and an installation  double team enabled us to extract the mass and keep the uterus. The postoperative course was uneventful. With a decline of 11 months, the patient is doing well and is in her 17th week of pregnancy. The tumors of the broad ligament are rare and mostly benign. The challenge in young women remains the utero-adnexal preservation.
