Chronic Protracted Dislocation of the Temporomandibular Joint: Conservative and Surgical Management- A Report of 2 Cases
Abstract
Background: Chronic protracted dislocation worsens with time due to continuing spasms and progressive fibrosis and consolidation. The aim of this report was to document our experience in the management of two different cases of longstanding dislocation in our centre.
Method: First case was treated with traction method using slow elastic rubber traction with elastic bands placed on upper and lower arch wires and the second was treated by an inverted L ramus ostectomy surgical procedure.
Result: First patient was discharged after the retrusion of the mandible back into position and correction of the cross bite. Outcome was also satisfactory for the second case, although we noticed slight anterior open bite a week post-operative which was not apparent in the immediate post-operative period.
Conclusion: Applicable treatment varies from external to internal traction procedures and eventually surgical procedures depending on the duration. Type of surgery will depend on the dimensions of the ramus and extent of mandibular prognathism.
