Profil des méningites bactériennes pédiatriques au Centre Hospitalier Régional de Tsévié (Togo)

Abstract

Cette étude des méningites qui demeurent un problème de santé publique a été motivée par les difficultés diagnostiques de cette affection au Togo. Il s’agit d’une étude rétrospective portant sur les dossiers d’enfants hospitalisés entre le 1er janvier 2004 et le 31 décembre 2008 (5 ans), au CHR de Tsévié. Résultats : le nombre d’enfant hospitalisé était de 3 680, dont 48 pour méningites bactériennes, avec un tôt de prévalence de 1,3%. Les enfants de moins de 5 ans ont représenté 58,5% des cas, dont 60,7% de moins de 1 an. La coloration Gram a permis de visualiser dans 82,9% des cas, le germe en cause. Les germes identifiés étaient Haemophilus influenzae b (40%), Streptococcus pneumoniae (37,1%), Neisseria meningitidis (20%) et Cryptocoque (2,9%). Le taux de létalité était de 33,3%. Conclusion : cette étude a confirmé la gravité des méningites bactériennes pédiatriques et rappel ainsi, l’importance de la vaccination.

Paediatric bacterial meningitis remained a problem in the tropic. We investigated their epidemiological, clinical, bacteriological, therapeutical and prognosis aspects in a regional hospital of southern Togo. We conducted a retrospective study between 1st January 2004 and 31 st December 2008 (5 years). Results: 3 680 children were hospitalized, and 48 for paediatric bacterial meningitis, with a hospitalization rate of 1.3%. Children under 5 years were more accounted (58.5%), with 60.7% under 1 year. With Gram colouring, we identified in 82.9% of cases, the germ in cause: Haemophilus influenzae (40%), Streptococcus pneumoniae (37.1%), Neisseria meningitidis (20%) and Cryptococcus (2.9%) were the germs in cause. In 27% of cases, the germ was not identified. The antibiotherapy was principally done with cephalosporin of third generation (50%) and ampicilline (41.7%). The mortality rate was 33.3%. Conclusion: The gravity of paediatric bacterial meningitis is real, with a very high mortality rate; and recalled thus, the necessity of vaccination.
