Le medulloblastome de l’enfant : a propos d’un cas et revue de la litterature recente internationale

Abstract

Le médulloblastome de l’enfant est une tumeur primitive neuro-ectodermique localisée au niveau de la fosse cérébrale postérieure. Il s'agit d'une lésion tumorale maligne du cervelet, de grade IV de l'OMS. Les auteurs rapportent le cas d’un médulloblastome chez un garçon de 11 ans référé de l’hôpital de Nouakchott au CHU FANN de Dakar. Le diagnostic était scanographique mais l’IRM est meilleure en ce qui concerne le diagnostic et le suivi pronostic. Le traitement est à base sur la chirurgie, la radiothérapie, la chimiothérapie et le pronostic du médulloblastome semble beaucoup s’améliorer grâce à ce protocole. L’Afrique connaît encore d’énormes difficultés pour l’application de ce protocole du fait, d’une part, du plateau technique insuffisant et de la paupérisation de la population d’autre part. En revanche, malgré ces difficultés , tous les espoirs nous sont encore permis grâce au programme neuro-oncologie pédiatrique initié par l’équipe de neuro-pédiatrie de Dakar.

English title: Children's medulloblastoma: about a single case study and recent international literature review

Summary

Children medulloblastoma is a primary neuro-ectodermal tumor located in the posterior fossa of the brain. It is a malignant tumor lesion of the cerebellum, WHO grade IV. The authors report the case of a medulloblastoma in a 11-year-old boy referred from Nouakchott hospital to CHU FANN in Dakar. The diagnosis was CT scan but MRI proves better in terms of diagnosis and follow-up prognosis. The treatment is based on surgery, radiotherapy, chemotherapy and the prognosis of medulloblastoma seems to improve a lot with this protocol. Africa is still experiencing enormous difficulties in applying this protocol due, on the one hand, to the insufficient technical platform and the pauperization of the population, on the other. However, despite these difficulties, we still have great hopes with now the pediatric neuro-oncology program initiated by Dakar neuro-pediatrics team.
