Hematome intracerebral probablement lie a l’isotretinoïne : A propos d’un cas

Abstract

Introduction : Nous rapportons un cas d’hématome cérébral survenu au cours d’un traitement anti acnéique par isotrétinoïne orale chez une jeune femme.
Observation : Une jeune femme de 23 ans, étudiante, sans facteur de risque cardiovasculaire, a présenté un hématome lobaire occipital droit après  2340 mg d’isotrétinoïne orale pour une acné récalcitrante au traitement habituel. Les explorations à visée étiologique n’ont pas révélé de malformation vasculaire, ni d’autre cause d’hémorragie lobaire du sujet jeune. Le score d’imputabilité de Naranjo était estimé à 6.
Conclusion : L’évaluation du risque vasculaire devrait être proposée avant tout traitement par isotrétinoïne orale. La particularité de ce cas clinique réside dans la rareté de l’hématome cérébral comme effet adverse du traitement par isotrétinoïne orale.

English Title: Intracerebral hematoma probably related to isotretinoin: A case-report

Introduction: We report a case of cerebral hematoma that occurred during an oral acne treatment with isotretinoin in a young woman.
Case report: A 23-year-old female student with no cardiovascular risk factor had a right occipital lobar hematoma after 2340 mg oral isotretinoin for recalcitrant acne with usual treatment. The etiological investigations did not reveal any vascular malformation nor any other cause of lobar hemorrhage of the young subject. Naranjo's accountability score was estimated at 6.
Conclusion: Vascular risk assessment should be proposed before any treatment with oral isotretinoin. The peculiarity of this clinical case lies in the rarity of the cerebral hematoma as an adverse effect of treatment with oral isotretinoin.
