Diagnostic du nanisme thanatophore a la clinique principale de l’Association Togolaise Pour le Bien-Etre Familial (ATBEF) : a propos de deux cas

Abstract

Le nanisme thanatophore est une ostéochondrodysplasie due à une mutation du gène FGFR3 (fibroblast growth factor receptor 3) localisé sur le bras court du Chromosome 4. Il est caractérisée par une macrocranie, un raccourcissement des membres, un thorax étroit et un abdomen proéminant reconnu, en anténatal à l’échographie et à la radiographie du contenu utérin. Ce diagnostic peut également être évoqué en post natal par l’examen clinique et la radiographie du nouveau-né qui met en évidence les mêmes éléments. C’est une malformation létale rare. Le dépistage anténatal précoce permet de proposer une interruption thérapeutique de la grossesse à la gestante dans le but de lui éviter tout traumatisme psychologique et obstétrical.

Nous rapportons deux cas de nanisme thanatophore dont le diagnostic a été évoqué en anténatal et en post natal au travers desquels nous précisons les modalités du diagnostic, discutons de la prise en charge et apprécions la répercussion psychologique.

English Title: Diagnosis of dwarfism thanatophoric in the principal clinic of ATBEF in Lome, Togo: a report of two cases

English Abstract

The Thanatophoric dwarfism is a osteochondrodysplasia due to a mutation of the gene FGFR3 (fibroblast growth factor receptor 3) located on the short arm of chromosome 4. It is characterized by macrocranium, a shortening of the limbs, narrow thorax and abdomen proéminant recognized prenatally by ultrasound and radiography of the uterine contents. This diagnosis can be evoked in post natal by clinical examination and radiography of the newborn that highlights the same elements. This is a rare lethal malformation. Early antenatal screening can provide a therapeutic abortion to the pregnant in order to avoid physical and psychological birth injury. We report two cases of dwarfism Thanatophoric whose diagnosis was made in antenatal and post natal through which we specify the terms of diagnosis, discuss the care and appreciate the psychological impact.
