Anevrisme mycotique tuberculeux de la carotide commune gauche au Benin

Abstract

L’anévrisme mycotique tuberculeux est très rare et n’a jamais été décrit comme une atteinte extra pulmonaire de la tuberculose au Bénin. Nous rapportons ici le cas d’une jeune femme de 25 ans présentant des conglomérats d’adénopathies cervicales chez qui le diagnostic de tuberculose ganglionnaire été posée par l’anatomopathologie.

L’apparition trois mois après le début de la symptomatologie d’une masse latéro cervicale gauche pulsatile et rapidement progressive a motivé la réalisation d’une échographie doppler cervicale puis celle d’un angioscanner des troncs supra aortiques qui ont conclu à un pseudo anévrisme mycotique de la carotide commune gauche.

L’anévrisme mycotique tuberculeux carotidien est rare. L’échographie doppler des vaisseaux du cou et l’angioscanner des troncs supra aortiques constituent la pierre angulaire du diagnostic des anévrismes infectieux.

English Title: Tuberculous mycotic aneurysm of the left common carotid artery, Benin, West Africa

English Abstract

Mycotic tuberculous aneurysm is very rare and has never been described as extrapulmonary tuberculosis in Benin country. We report the case of a 25-year-old woman with conglomerates of cervical lymphadenopathy diagnosed with lymph node tuberculosis by anatomopathology. The appearance of a pulsatile and rapidly progressive left lateral cervical mass three months after the beginning of the symptomatology motivated a cervical doppler ultrasound and a CT angiography of supra aortic trunks help to diagnose a pseudo mycotic aneurysm of the left common carotid.

Mycotic carotid tubercle aneurysm is a rare disease. Doppler ultrasound of the vessels of the neck and CT angiography of supra aortic trunks are the cornerstone of the diagnosis of infectious aneurysms.
