Chirurgie gratuite de la cataracte en strategie avancee a Lome (Togo): quel gain visuel?

Abstract

Objectif: Décrire les résultats fonctionnels de la chirurgie gratuite de la cataracte en stratégie avancée.

Matériels et Méthode: Etude rétrospective analytique des dossiers de patients opérés du 08 au 24 janvier 2015 à 15km de Lomé. Etaient inclus tous les patients adultes non monophtalmes opérés de cataracte (acuité visuelle pré opératoire de loin inférieure à 1/10) par phacoalternative et suivis sur une période de plus d’un mois. Le jour suivant l’intervention, les opérés étaient revus sur place et les autres rendez-vous étaient donnés dans un même centre ophtalmologique de Lomé. Les résultats fonctionnels post opératoires ont été classés selon les normes de l’OMS en trois groupes d’acuité visuelle (AV) : le bon résultat (AV [10/10-3/10]), le moyen (AV [1/10-3/10[), et le mauvais résultat (AV<1/10).

Résultat: Des 89 (64,49%) opérés, l’âge moyen était de 56,29 ± 15,72 ans [15 et 95] et le sex-ratio était à 0,74 en faveur des femmes. 81 yeux (91,01%) avaient une AV pré opératoire < 1/20 dont 56 yeux (62,92%) percevaient la lumière. Les complications post opératoires à J1 étaient dominées par l’oedème cornéen (20,22%), des buées épithéliales (3,37%) et une anomalie de la pupille (2,25%). A trois semaines (S3) l’oedème cornéen était la complication la plus fréquente (18,03%), suivi des anomalies de la pupille (9,84%). L’opacification capsulaire postérieure était la principale complication dans 5,56% à la 6^ème semaine. A S3, 54,10% des patients avaient une bonne AV au trou sténopéique (TS) (≥3/10), 8,20% avaient une AV limite (1/10 à 2/10) et les patients avec une complication avaient une moins bonne AV au TS (p=0,000). 63,89% avaient une bonne AV corrigée au TS (≥3/10) à la 6^ème semaine.

Conclusion: La chirurgie de la cataracte en stratégie avancée a permis de sortir de la cécité des personnes ayant une faible accessibilité aux soins oculaires. Le renforcement du suivi post opératoire permettra l’amélioration du résultat fonctionnel.

English Title: Free eye camp strategy in Lome (Togo): what visual gain?

English Abstract

Objective: To describe the functional results of cataract free surgery in advanced strategy.

Materials and method: Analytical retrospective studies of cases of patients operated 15 km far from Lomé from January 8 through 24, 2015. Those included are any patient adult non monophtalmic, operated for cataract (preoperative acuity considerably lower than 1/10) by phaco-alternative who was seen again at least one month after. The day following the intervention, the patients operated were seen again on site and the other appointments were given in the same ophthalmologic center in Lomé. Postoperative functional results were classified according to WHO standards into three groups of visual acuity (VA): the  good result (VA [10/10-3/10]), the average (VA [1/10-3/10]), and the bad (VA<1/10).

Results: Out of the 89 (64.49%) patients operated, the average age was 56.29 ± 15.72 years [15 and 95] and the sex ratio was 0.74 in favor of the female. 81 eyes (91.01%) had a preoperative VA < 1/20, among them 56 eyes (62.92%) could perceive light. Postoperative complications on D1 were mainly corneal oedema (20.22%), epithelial mist (3.37%), and an anomaly of the pupil (2.25%). At week three (W3) the corneal oedema was the most frequent complication (18.03%), followed by the anomalies of the pupil (9.84%). The posterior capsular opacification was the main complication in 5.56% at week 6. At W3, 54.10% of patients had a good VA at the stenopaeic hole (≥3/10), 8.20% had an average VA (1/10 to 2/10), and patients with a complication had a poor VA at the stenopaeic hole (p=0.000). 63.89% had a good VA corrected at the stenopaeic hole (≥3/10) at week 6.

Conclusion: Cataract surgery in advanced strategy enabled to free from blindness people with little access to eye care. The strengthening of the postoperative follow-up will enable improvement of the functional result.
