Abstract

Background: Enterobacteriaceae urinary tract infections (UTIs) are very common in the community and in the hospitals.

Objectives: The objectives of this study are to determine the hospital prevalence of Enterobacteriaceae UTIs, to describe the factors associated with these infections and to evaluate the antibiotic resistance of these bacteria.

Methods: It is an analytical cross-sectional study of 524 results of cytobacteriological examination of urine (CBEU) in a period of eight months from January 2022 to August 2022 in the University Hospital of Befelatanana in Antananarivo, Madagascar.

Results: Among of the 524 results of CBEU, 106 cases were represented by isolates of Enterobacteriaceae showing a prevalence of 20.2% of Enterobacteriaceae UTIs. Among these Enterobacteriaceae, Escherichia coli were the most common representing 75 (70.8%) cases. Concerning the associated factors, women (21.9%) (p=0.24; NS), subjects less than 20 years (29.8%) (p=0.000035), with urinary disorders (26.7%)(p=0.00031) and hospitalized in infectious diseases department (44%)(p=0.00011) were the most affected by Enterobacteriaceae UTIs. Concerning the antibiotic resistance of Enterobacteriaceae, it varied between 5.3% (Imipenem) to 92% (amoxicillin).

Conclusion: In brief, we must improve the care of vulnerable patients with UTIs and limit the use of broad-spectrum antibiotics in hospitals to control the spread of multi-resistant Enterobacteriaceae.
