Tumeur cutanée historique négligée révélatrice d’un lymphome B Neglected historic skin tumor revealing lymphoma B

Abstract

Giant skin tumours are defined as greater than 10 cm in size and are frequently a consequence of neglect. We report the case of a 53-year-old woman who had a giant diseased and infected skin tumor in the right arm since 3 years. The delayed skin biopsy revealed cutaneous lymphoma B with unusual presentation. The denial of the disease due to fear of the diagnosis explains the late consultation and the pejorative evolution (upper limb amputation) of the case. Multidisciplinary management (involving the psychiatrist, clinical psychologist, dermatologist, surgeon, hematologist, etc.) is crucial to improve the prognosis of this pathology.

Les tumeurs dites « historiques », ou tumeurs négligées, sont des tumeurs géantes de diamètre habituellement supérieur à 10 cm et de longue durée d’évolution. Nous rapportons le cas d’une femme de 53 ans présentant une tumeur cutanée géante infectée du bras droit évoluant depuis 3 ans. La biopsie cutanée réalisée tardivement a révélé un lymphome cutané B de présentation inhabituelle. Le déni de la maladie explique en grande partie la consultation tardive et l’évolution péjorative (amputation du membre) du cas. La prise en charge multidisciplinaire (impliquant le psychiatre, psycho-logue clinicien, dermatologue, chirurgien, hématologue etc.) est cruciale pour améliorer le pronostic de cette pathologie.
